Pre-existing Medical Insurance

My mother which is now deceased found out after a hospital stay that she was not covered because of a pre-existing condition. My question is: Does a person receive documentation fully explaining that if a pre-existing condition exist that they will not be covered until the allowable time has been reached? Because of this she was not able to maintain the periodical treatment which in return resulted in her death. She was a resident of Illinois.
