Weaning off of Prempro

I have been using Prempro very successfully to cure hot flashes which resulted in sleepless nights. I remember my doctor saying that to discontinue using Prempro, you have do it slowly but I never went into details. What is the best way to stop taking it. I've been on it for a number of years and am concerned about the side effects.
