Demerol injection and phlebitis

Dear Sir/Madam

I've been taking demerol intravenously since last three years, after a deadly accident nearly amputating my leg. although my leg is healed via bone grafting, i still experience intense pain perhaps because of peroneal nerve entrapment in my callus of fibula. I forgot to state that i'm a primary care physician. and by the way also i suffer from migraine attacks once a month (probably it's a catamenial migraine) i often take demerol every 4-5 weeks and now i realize that all my veins are fibrotic!!!! and infact i developed phlebitis. would you please kindly recommend me a way to reduce this phlebitis or stop injecting demerol. i'm not physically, but to a mild extend psychologically dependent to this medication.
