Use of Actiq for non-cancer related diagnoses

Is Actiq prescribed by physicians whose patients are seen for pain management and who have not been diagnosed with cancer? Is it true that the FDA precludes coverage of the drug for diagnoses other than cancer? The patient in question is on Medicare, has Part D coverage, but is being denied the drug by their prescription drug plan because she has a nerve disorder and does not have cancer.
