Action/Kinetics: Produced by Streptomyces spectabilis. It inhibits bacterial protein synthesis by binding to ribosomes (30S subunit), thereby interfering with transmission of genetic information crucial to life of microorganism. Mainly bacteriostatic. Only given IM. Peak plasma concentration: 100 mcg/mL (2-g dose) after 1 hr and 160 mcg/mL (4-g dose) after 2 hr. t 1/2: 1.2-2.8 hr. Not significantly bound to protein. Excreted in urine.

Uses: Acute gonorrheal proctitis and urethritis in males and acute gonorrheal cervicitis and proctitis in females due to susceptible strains of Neisseria gonorrhoeae. For pharyngeal infections, use only in those intolerant to cephalosporins or quinolones. It is ineffective against pharyngeal infections and against syphilis; is a poor drug to choose when mixed infections are present.

Contraindications: Sensitivity to drug.

Special Concerns: Safe use during pregnancy, in infants, and in children has not been established. Benzyl alcohol in the product may cause a fatal gasping syndrome in infants.

Side Effects: A single dose of spectinomycin has caused soreness at the site of injection, urticaria, dizziness, nausea, chills, fever, and insomnia. Multiple doses have caused a decrease in H&H and C CR and an increase in alkaline phosphatase, BUN, and ALT. In single or multiple doses, decrease in urine output.

How Supplied: Powder for injection: 2 g

?IM Only Gonorrheal urethritis in males, proctitis, and cervicitis.
2 g. In geographic areas where antibiotic resistance is known to be prevalent, give 4 g divided between two gluteal injection sites.
Alternative regimen for urethral, endocervical, or rectal gonococcal infections in clients who cannot take ceftriaxone.
Adults and children weighing more than 45 kg: Spectinomycin, 2 g, as a single dose followed by doxycycline. Children weighing less than 45 kg: 40 mg/kg given IM once.
Gonococcal infections in pregnancy where client is allergic to beta-lactams.
2 g followed by erythromycin.
Disseminated gonococcal infection where client is allergic to beta-lactams.
2 g q 12 hr.
