Classification: Digestant

Action/Kinetics: Enzyme concentrate from hog pancreas, which contains lipase, amylase, and protease, enzymes that replace or supplement naturally occurring enzymes. More active at neutral or slightly alkaline pH. Has 12 times the lipolytic activity and 4 times both the proteolytic and amylolytic activity of pancreatin. Certain products have an enteric coating that protects the enzymes from deactivation in the stomach.

Uses: Pancreatic deficiency diseases such as chronic pancreatitis, cystic fibrosis of the pancreas, pancreatectomy, ductal obstructions caused by cancer of the pancreas or common bile duct, steatorrhea of malabsorption syndrome or postgastrectomy or postgastrointestinal surgery. Presumptive test for pancreatic function, especially in insufficiency due to chronic pancreatitis.

Contraindications: Hog protein sensitivity. Acute pancreatitis, acute exacerbation of chronic pancreatic disease.

Special Concerns: Safety for use during lactation and in children less than 6 months of age not established. Methacrylic acid copolymer, which is found in the enteric coating of certain products, may cause fibrosing colonopathy.

Side Effects: GI: Nausea, diarrhea, abdominal cramps following high doses. Other: Inhalation of the powder is irritating to the skin and mucous membranes and may result in an asthma attack. High doses cause hyperuricemia and hyperuricosuria.

Overdose Management: Symptoms: Diarrhea, intestinal upset.

How Supplied: Capsule; Enteric Coated capsule; Enteric Coated tablet; Powder; Tablet

Adults and children over 12 years of age: 4,000-48,000 units of lipase with each meal and with snacks. Children, 7-12 years: 4,000-12,000 units of lipase with each meal and snacks; 1-6 years: 4,000-8,000 units of lipase with each meal and 4,000 units lipase with snacks. 6-12 months: 2,000 units lipase with each meal. Dosage has not been established in children less than 6 months of age. Severe deficiencies may require up to 64,000-88,000 units of lipase with meals (or the frequency of administration can be increased if side effects are not manifested).
Pancreatectomy or obstruction of pancreatic ducts.
Adults: 8,000-16,000 units of lipase at 2 hr intervals or as directed by a physician.
Cystic fibrosis.
Use 0.7 g of the powder with meals.
