has anyone had an endometrioma or a chocolate cyst? If so, what were
your symptoms and what was the treatment? Mine is 5cm so
far.....started BC pills this week....

Hey mommy0moo, how are you doing? The goal of directed medical
treatment is to achieve an anovulatory state. This can also be
accomplished with progestational agents (ie, medroxyprogesterone),
danazol, gestrinone, or gonadotropin-releasing hormone agonists
(GnRH), as well as other less well-known agents if oral contraceptives
and NSAIDs are ineffective in controlling symptoms. Endometrosis
and other gynecologic disorders hardly ever manifest themselves by
back pain. Menstrual pain is poorly localized, tends to radiare down
the legs and is of a crampy nature. Endometrosis or carcinoma of
the uterus (body or cervix) may invade the uterosacral ligaments.
Malposition of the uterus may pull on them. Pain is localized
centrally in the sacrum below the lumbosacral joint, but may be more
on one side. Pain begins during the premenstrual phase, often
continues until it merges with menstrual pain. Malposition of the
uterus (retroversion, descensus, and prolapse) leads to sacral pain -
especially after the patient has been standing for serveral hours.
Carcinomatous pain implication of nerves plexuses is continuous and
becomes more severe as it progresses. Endometrosis tends to be more
intense at night. Pain in the back may be consequense of arthritis of
the cervical of thoracic spine. GnRH can be combined with estrogen
and progestogen (add-back therapy) without loss of efficacy but with
fewer hypoestrogenic symptoms. No evidence exists that any one of
these agents is most effective in controlling pain; moreover, there is
no strong evidence that they improve fertility. Conservative
surgery can be performed to preserve fertility in young patients.
Definitive surgery is a hysterectomy and bilateral oophorectomy.

Thank you so much for your reply. This has been a horrible couple of
weeks - this pain is worse than labor - seems as if there is no end!!
I go in for another U/S on Thursday - the cyst potentially ruptured
over a week ago and have been bleeding for 9 days straight. I am
exhausted, dizzy and weak. Nightime is definitely worse - cannot
sleep for the excruciating pain and swelling.... I am ready to
'leap up on the table' hysterectomy sounds really nice right about
now. I'm only 31, but have not been able to conceive for 3 years -
PCOS, Insulin resistant, 1 sucessful pregnancy 5 years ago, 1 m/c 3
years ago in May. Nurse told me she thinks that my cyst is leaking
so they will look to see if that's the case. I hope if anyone else
has had this type of cyst that they can share their experience and
treatments. I will search eMedicine now - thank you so much for the
advice - Iwill also ask about the meds you suggested. Thank you
again.... :D

Hi mommy0moo! I found out this past Monday that the left ovarian
cyst I have been taking BC pills for the last 6 weeks is most likely
an endometrioma. I first saw my gyno at the end of February because of
pelvic pain on my left side, back pain (I have a tilted uterus), and
intense periods (like you said, "worse than labor"...even having BMs
is excruciating). I had a vaginal ultrasound, and at that time, the
cyst was 5cm. He prescribed BC pills (high estrogen), but they didn't
stop its growth (it's 7cm now). My doctor now wants to do a
laparoscopy and probably a laparotomy to remove the cyst and the
ovary. I wasn't sure if I wanted to research it much on the Internet,
because there is a lot of misinformation out there, but just decided
today that I had to. I hate feeling uninformed. As I suspected I
would, since starting the research, I have more questions than ever!
From what I'm reading, these things can be malignant (although
unlikely in our age group) and a CA-125 blood test can help determine
if it is. Also, some doctors are able to remove the cyst and ovary
through incisions much smaller than the one my doctor is talking about
using with the laparotomy. Like you, I'm relatively young at 34.
My doctor asked me if I wanted him to remove my uterus, too. Even
though I've had three children and my baby-making days are over, I am
so torn about having the uterus gone. My mother had hers removed at my
age (same problem), and had "intimacy problems" ever since. I'm
sorry, I know I don't have any helpful advice for you right now. I
have more questions than answers! It sounds like we're in the same
boat. I just wanted to reply to your message to let you know you're
not alone. On your last post, you said you had been bleeding for
nine days. I started bleeding 1 week into my last pill pack, and bled
for 5 days. I'm still spotting, but my doctor never mentioin that the
cyst could be leaking. Let me know how your last ultrasound went.

I am really young to, I am 29, and had a hysterectomy 2 years ago, and for about three months I felt wonderful. I have had unbearable pain. I have gone to the hospital for months and last night they finely gave ultarsound and said I had chocolate cyst, the pain has gotten so bad I wonder if ruputred and does anyone know if that is dangerous?

Yes, I had a chocolate cyst. It grew very rapidly, in 5 months or less and at the time of my removal (3.5 weeks ago) it was 5 cm. I caused a great deal of pain on my left side because it was not only attached to my ovary, but also to my colon.

CrazyI am a 35yr old women who has been dealing with the chocalate endomitrioma since Feb. 2006 I finally had it removed in July just to have the pain return and a lump reestablish it self basicaly in the same area.. Has anyone had this problem? I am in so much pain and when put on BC I became severly depressed, and had high levels of fatigue. AHHH Im going nutz over this whole endometrioma ordeal, So everyone else who has or is going through it too, My sypathay goes out to you!Unsure

I had a cyst which was noticed on the ultrasound when i first got pregnant. The cyst seemed to have disappeared in later pregnancy as it did not show up on the ultrasound. The pregnancy went well and i delivered a year ago through caesarian section for other reasons - however during the operation it was noticed that the cyst was still present and was removed together with the ovary. Lab tests indicated that this was a 10cm chocolate cyst.Following the delivery (circa 1 month later) I started undergoing treatment so that hopefully chocalate cysts will not develop on the other ovary. This consists of the 'pill' which i am taking every single day of the month - this stops menstruation completely.

I was trying to do some resarch on chocolate cysts in pregnancy in order to assess the chances of success of having another baby, but found nothing. I would love to be directed to some website where i can read about this condition or perhaps some other browsers who can share the same or similar condition.

Hi Tess, I had a 10 cm chocolate cyst removed 2 years ago, and it's now back, 6cm and growing! It's on my left ovary, and really painful, so am going to have the ovary and cyst removed shortly. I had a hormone treatment after the last removal to stop it coming back, guess that wan't much good.....!

I am 34 and I have Endometriosis. In Dec 2005 I had a 10cm cyst drained. I appeared 6 months again 12cm. I was put on B/C but had not taken it. I have been taking Fibrovan and I have had no pain at all but yesterday I want to the doctor and found out that I have a 15cm cyst again. I am now taking danazol. the Fibrovan stopped all my pain but it does not stop the period so the cyst continues to grow.

I had an endometrioma (chocolate cyst) on each ovary about 10 years ago. I was in a lot of pain, had prolonged bleeding during menstruation, and was having difficulty conceiving. I had surgery. On one side the cyst was removed with a portion of the ovary, and on the other side the cyst, ovary and tube had to be removed (that cyst was 15 cm and engulfed the ovary). My doctor was skeptical about conception after surgery because I have "more scar tissue from endometriosis" than he had ever seen. Good news, six weeks after surgery I conceived our third child, with less than 1/2 an ovary.

Eight years later I was diagnosed with another cyst on my remaining ovary (9 cm and holding for 18 months now). I'm not in pain, so we've been monitoring it with ultrasounds every 6 months. My doctor wants to do surgery to "be safe". In fact, he wants to do a complete hysterectomy. I'm not in full agreement with his recommendation, so I'm researching. I would prefer to keep my uterus and cervix.

I just got out from the hospital yesterday. I had a 6cm cyst and the gyne advised to remove it soon before it ruptures. Good thing they did because my cyst had implants on my uterus already. Terrible really to be having all these. Good thing you have a baby. Dont be scared, I'm only 19 yet I dont have my left ovary and there are implants on my uterus. I woory I might not get pregnant anymore. Be strog and do undergo surgery.

Yes. I just had a chocolate cyst removed about 5 or 6 weeks ago. Mine had grown to about 15 lbs. I woke up that morning with this sharp pain. Something that felt like I was being stabed repededly. The pain eventually became so severe and constant that I had to go to the hospital. I had to have emergency surgery that same day. The cyst had surrounded my intestance and my apendex. And had also pushed my hip out of place. The surgery took about 2 hours and I was in recovery for about 6 days.

Frown Hello I am a forty yr old who had a hysterectomy 4 yrs ago and just found out yesturday that I have a cyst over 4 cm filled with enometriosis I have had pelvic pain all my life yada yada yada I am actually so tired of complaining about it anyway I have to get surgery on Monday and have my ovaries removed then they are going to check my bladder because it is laying on my bladder hopefully it did not attach yet and then my surgeon who did my bowel obstruction surgery 4 yrs ago also is going to be present to check my colon, so if there is anyone out there that can help me a little I would be grateful because I am ready for a nervous breakdown. Thanks

I had a chocolate cyst at the age of 17..it was really painful and it was 8-10 cm...i can't attend school because of this...good thing i had it removed last september..i was just depressed that i had to experience a surgery at an early age...and it also gave me an ugly scar...

I'm 27 and had a 10cm cyst removed form left ovary 1yr ago. Slow recovery and pain has returned i'm scared its started up again. Ive been put on BC but hate them.

Hay I have been diaganosed with endo since I was sixteen I have a very severe case I lost my left ovary to a larger painful grapefruit sized endometrioma I ahve had 4 surgeries and just found out it has spread again another endometrioma right ovary will be haveing it removed along with the ovary it is again huge about the size of a softball I know how it feels and it sucks All i count on is the the hope one day I will be completey pain free Hope is good Bless ed Be

I am 43 and have endometriosis since I was about 18, but at the time they never knew what it was. I've had 5 laporoscopies to remove endo and it just continues every 5-6 yrs. Have had cysts, severe pain, endo around bowels, bladder and plenty of scar tissue. I now have a endometrioma and am getting ready to have a complete hysterectomy. I am terrified now loooking at all the complaints and am like you just over it all. I fear that it will only continue like others we read about and wonder where it will attach itself next if nothing is there. Another big fear for me is the hormone replacement therapy. I hope to be rid of pain and not regret my surgery decision. Reading these things is probably a bad thing for all of us sufferers. I want a normal life, and sex life and not feel like a freak with this pain that is undescribable, but feel forced to have the surgery after being on many meds and not work just horrible side affects.

I am in about the same boat...I am 42 with endo since teen years and facing a complete hyst. Lucky I was able to have two kids, as I was advised in my early 20s to not delay getting pregnant. I had major surgery to remove the endo at age 24 (then went on Lupron for 6 months) and got preg at age 26, and 28. I have had many lap. (maybe about 5 or 6) and each time, the endo is there. I had an ablation 3 years ago and that solved the heavy bleeding, but that's it. I have been on b/c for 6 months and that's helped a little, but I have a grapefruit size choc cyst on my left ovary. Dr. said today that the b/c is just like putting a bandaid on the endo. He said that a total hyst is almost definitely in my future. I am just so worried about the side effects. Early aging, the heart stuff, and then the full-blown menopause.

Design: Controlled in vitro study. Setting: Department of Obstetrics and Gynecology. State University of New York Health Science Center. Patient(s): Five women with ovarian endometriomas. Intervention(s): Endometrioma tissue and chocolate fluid from five different patients were entered in this study. Main Outcome Measure(s): Endometrioma cell proliferation in culture with and without chocolate cyst fluid. Result(s): Chocolate cyst fluid increased the proliferation of endometrioma cells compared with controls. Also, high concentrations of transforming growth factor-B1 were found in cysts' fluid.
