My name is lee. I am 44 years old and I wanted to ask for guidance. My family practice doctor does not provide good health care any longer. Nice guy but getting older and forgets things al ot. Likes to blame everything on most of his patients on depression. I have seen a psych and I am not depressed. I have had Swelling (non painful) in several lymph areas. It started about 6 months ago on the left side of my face under my ear, jaw and neck. Now I have swelling on the left side of my inner thigh (groin area, i guess), and under my arms. Late last year I had two separate scrotal abcesess which were pretty bad. I was out of work for almost 3 weeks after they drained them. They had to be packed 3 times a day. My urologist thought I was too young and healthy to be getting this type of abcess. I was also treated for prostitis and urinary retention. I have never had health problems until the past year. I had an MRI about 8 months ago which showed a 3cm mass in the lower pole of my left kidney with low signal on t1 and high on t2 probably filled with protein or blood. I just had another mri a month ago (due to ongoing pain in my lower back and legs) This time the mass showed high signals on t1 and low on t2 (the opposite as before) the radiologist wrote that due to the atypical signals and due to the fact that calcium was noted in the cyst on a previous CT scan (about 10 months ago) that the mass should be followed up on. My doctor does not seem to think so. I was just wondering if anyone knew anything about this stuff. I have pain in my pelvic area, legs and I am fatigued alot, no appetite. I don't know if this swelling that I have could be related. My family doctor examined me (after I suggested it) and noted the swelling but did not say anything about it. I also had a brain scan about 8 months ago (I was having what they called cranial nerve deficits on my left side) the MRI showed abnormal high signal intensity in both frontal lobes and right occipital lobe deep within the white matter too numerous for age. the "lesions" had no specific distribution or appearance.
Just curious if any of this is related or am I just falling apart. My family practice doctor was not sure if he could feel any lymph nodes in the swollen areas or not.

Skip the family doctor and call and make an appointment with a Vascular Surgeon. You will receive the proper follow up to your symptoms. When you make the appointment, tell them you want a consult for swollen lymph nodes. They should get you in within 2 weeks. Also, contact the radiology department of your hospital and ask them for a cd of your MRI's and your CT. The reports are burned right onto the cd with the films. Hand carry this to your appointment with the surgeon (so it doesn't get lost in the process). If you have had any recent labwork, which I am assuming your doctor was intelligent enough to order, request those reports as well. Don't delay in making this appointment. Most general surgeons do vascular consults so if you don't see the "Vascular" title when searching your Yellow Pages, call a general surgeon's office in your area. You can get good help from one of them!

Thank you very much. Do I need a cousult for a vascular surgeon? My doctor probably won't give me one. He did run labs about 5 weeks ago and their office policy is do not call on your lab results. If anything shows up the doctor will call you or they will mail the results within the week. I have not received a phone call or anything in the mail. Not unusual after my last MRI, he had the results for a month and than just mailed them to me with a note saying sorry it took so long and please remind me in 6 to 8 months that we might follow up on the kidney mass. I appreciate your help. What could be vascular?

Oh also wanted to mention there is swelling (noticible in those areas but I cannot feel any nodes, some small bumps but nothing large). Prostrate cancer and serious heart problems run in the male side of my family. Could this be related? I did have some urological problems about a year ago. I was treated for prostitis, urinary retension and I develped a very bad scrotal area abcess that required an emergency incision and drainage, it came back 5 weeks later and was drained again. I missed work for 3 weeks, had visiting nurses coming and stuff. The urologist mentioned that he did not understand someone my age having these problems because my immune system should be stronger. You seem to know what your talking about. I really really appreicate it Still don't know if I need a consult or not to see a vascular surgeon.

I think a Vascular surgeon is your next step. You have unexplained swelling in various areas, and suffer from leg pain. These are signs of poor circulation. A vascular surgeon will assess you for circulatory problems. He will listen to the pulses in your legs, ankles and neck and will tell you if they are of good or poor quality. He can also order proper subsequent studies if necessary. They are good. Your family doctor told you he "isn't sure" if the swelling is lymph node-related or not. That is because it is not his speciality. He should be concerned enough to want this thoroughly checked out for you. A vascular surgeon can help with this. You don't always need a referral to see one, you can call and make this appointment on your own. However, because I don't know how your insurance works, you should first try to get a referral from your family doctor just in case this is the only route that your insurance will cover. Your family doctor shouldn't give you any trouble at all - it would be ridiculous for him to deny your interest in an opinion from a specialist. I'm sure he will give you the referral.

You have recurrent abscesses as well. Some people just develop these - it doesn't necessarily mean that there is anything wrong with you in that respect. They are infections caused by bacteria and bacteria is found in and on all of us. Sometimes however, abscesses can be indicative of an immune disorder but if that was your case, this would show up in your lab results which, I'm assuming were good or you would have been told otherwise. I see where your urologist said that he thought your immune system should be stronger than it is but you never said that it was proven that your immune system is weak. Remember that a Urologist is limited in specializing in Urological disorders/diseases. Also, you mention that you have a family history of protate cancer. For this reason, you should have your prostate checked on a yearly basis. Prostate cancer and your scrotal abscess are unrelated so don't worry about that. Keep your scrotal area as clean as possible and you can possibly help to prevent a recurrent abscess in that area.

You mention that you have a strong family history of heart problems/disease. Actually, most of us do! Are you short of breath? Do you have chest pain or heaviness? Do you sweat profusely upon slight exertion? Do you have swelling in your ankles and hands? The best thing you can do with a family history of heart problems is to make changes in your life to take better care of yourself. If you are overweight, try to take control of that and lose some if you can. If you are eating improperly, make a change there. Your fatigue could be due to poor eating habits, lack of sleep and stress. You are worried about a lot of things. You are worried about developing conditions that you don't even have. However, it's also understandable that you are worried because you don't have good answers yet.

You say you have a kidney mass and then later mention that it was a cyst. There's a huge difference between an unknown mass and a cyst. Many, many people have cysts on their kidneys. These come and go as well and aren't alarming. Your radiologist is recommending a repeat CT in a few months from now. Do it. It is to your benefit. Radiologists are the ones who read and interpret the images for your physician, not vise versa.

In regards to your obtaining your records... you have every full and legal right to your own medical reports. Just ask for them. You will have to sign a release for them but they belong to you. You can get these from your doctor's office or from the hospital where your studies were done. And, there is no reason why you should not be able to obtain these within a few days if not immediately.

THank you very much for taking the time to write back. It is all good advice. I am calling my family practice doctor today for a referral. Sorry confused you with the kidney mass/cyst. I have both on the same kidney one on the inferior pole was called a mass with atypical weighted signal changes on mri with previous calcification and the other one is only a cyst on the superior part of the same kidney. My dad has numerous cysts on his kidney. He had one I guess it is the mass with the changes that the radiologist seemed concerned about. Thank you again. Your reply helped me with the motivation to take some action.
