Classification: Emetic

Action/Kinetics: Acts both locally on the gastric mucosa as an irritant and centrally to stimulate the CTZ. The central effect is caused by emetine and cephaeline, which are two alkaloids in the product. Onset: 20 min. Duration: 20-25 min. In contrast to apomorphine, a second dose may be given if necessary. Ipecac syrup must not be confused with ipecac fluid extract, which is 14 times as potent. Syrup of ipecac can be purchased without a prescription.

Uses: To empty the stomach promptly and completely after oral poisoning or drug overdose.

Contraindications: With corrosives or petroleum distillates, in individuals who are unconscious or semicomatose, severely inebriated, or in shock. Infants under 6 months of age.

Special Concerns: Use with caution during lactation. If used in children less than 12 months of age, there is an increased risk of aspiration of vomitus. Abuse may occur in anorexic or bulemic clients and its use in these groups has been associated with severe cardiomyopathies and death.

Side Effects: Diarrhea, drowsiness, coughing, or choking with emesis, mild CNS depression, GU upset (may last several hours) after emesis. Can be cardiotoxic if not vomited and allowed to be absorbed. Cardiotoxic effects include heart conduction disturbances, atrial fibrillation, or fatal myocarditis.

Overdose Management: Symptoms: If absorbed into the general circulation, symptoms may include cardiac conduction disturbances, bradycardia, atrial fibrillation, hypotension, or fatal myocarditis. Treatment: Activated charcoal to absorb ipecac syrup. Gastric lavage. Support the CV system with symptomatic treatment.

Drug Interactions: Activated charcoal adsorbs ipecac syrup, thus decreasing its effect.

Dosage
?Syrup Emetic.
Adults and children over 12 years: 15-30 mL followed by 240 mL of water; infants up to 1 year: 5-10 mL followed by one-half to one glass of water; pediatric, 1-12 years: 15 mL followed by one to two glasses of water.
