Protamine reaction/anaphylaxis

Hello, 

I hope you can help. My Mother suffered a fatal reaction to protamine during a heart valve replacement surgery. She experienced severe hypotension an anaphylaxis. Her records indicate she was given doses of 100 mg, twice within a span of about 40 minutes. Would that be too rapid of administration? I hope you can give me a prompt response. Thank You
